Onboarding

EquiLoop makes it simple to connect community members with the local services that truly improve their health and well-being. From food and housing to behavioral health and beyond, our platform helps address the Social Determinants of Health (SDoH) that shape outcomes.

Onboarding is extremely straightforward.  

1. First we need to identify the Key Users and Admins. 

2. From there, we determine the Goals of the System.

3. Next, we identify the data points that need to be tracked and reported on.

4. Then, we list the Services that need to be added.

5. Screening questions and outcomes are identified.

6. Consent timing and management strategies are determined.
